Our mission is to design, develop and implement strategies to decrease the incidence of cardiac disease, stroke and related chronic disease in Northern New York State.
All our activities are aimed at reducing the top 2 causes of chronic disease - tobacco use & obesity.

North Country Tobacco Cessation Center
Our tobacco dependence specialists can help you if you are:
- A healthcare provider interested in incorporating evidence based interventions in your practice.
- An Institution interested in adopting a tobacco free policy.
- Concerned about your own tobacco use
- Concerned about a family member's or friend's use of tobacco.
Eat Well Play Hard North Country Coalition
This portion of our website is particularly helpful if you are :
- An administrator of an organization that serves young children – 2 to 12 years old
- An institution interested in developing, revising or expanding a Wellness Policy regarding nutrition and/or physical activity
- An after school program seeking to enhance current physical activity programming and appropriate food offerings
- A childcare provider interested in enhancing age appropriate physical activity
- Interested in applying for an Eat Well Play Hard Mini Grant
- Concerned about your family's health.
